Outreach & Prevention

Outreach and Prevention Program staff takes calls from homeless or imminently homeless individuals with the goal of stabilizing them in current housing or assisting them in finding safer living conditions. Home of the Sparrow offers linkages to services including emergency assistance with food, health care, clothing and transportation.

Additionally, linkages are offered for behavioral health services, legal assistance, mediation with landlords and more. A small pantry stocked with ready to eat foods, personal hygiene items, bottled water, first aid supplies, underwear and socks is available for callers needing immediate assistance.

Criteria for Program

– At risk for homelessness, currently homeless, those on our Transitional Shelter waiting list
– In need of short-term case management support, referral information, and advocacy.

Range of Services

Case management –Home of the Sparrow’s experienced Case Managers work with clients on areas of focus including employment, financial skills, housing, physical and mental health and well-being.  Regular case management involves support and guidance in achieving individual goals that will most immediately impact the housing circumstance.   The process serves to empower women as they transition from homelessness to self-sufficiency.

Referrals – Community Resources in McHenry County governmental and non-profit agencies offer an array of supportive services that strive to meet the needs of area residents.  Home of the Sparrow Case Managers are skilled at appropriate referral and utilization of those resources to supplement the supports offered by the agency.

Advocacy –Navigating the world of social services can at times be difficult and frustrating.  HOS staff assists clients with connections to services and, when needed, act as a strong voice on their behalf.
